Cách điền hoàn chỉnh Form W8

W8 là thứ không thể thiếu đối với người chơi Affiliate. Muốn nhận first payment của các network các bạn phai mail, fax hoặc email nó cho các network. Dưới đây là hướng dẫn hoàn chỉnh cách điền W8 dành cho cá nhân

Các phần đầu không điền gì cả, bắt đầu điền từ đây:

Part I: Identification of Beneficial Owner (See instructions.)

1. Name of individual or organization that is the beneficial owner

2. Country of incorporation or organization

3. Type of beneficial owner: các bạn tích vào ô Individual

4. Permanent residence address (street, apt. or suite no., or rural route). Do not use a P.O. box or in-care-of address.

City or town, state or province. Include postal code where appropriate.
